Financial Philosophy
Summit Oral Surgery (whose parent company is Summit Oregon LLC), shares the concern of our patients about the increasing cost of dental and medical care and ability to make those financial commitments. Our fees are comparable to the usual and customary charges made by Oral and Maxillofacial Surgeons in the area. These charges are based on the doctor’s costs, time, and skill involved. You will be given a written estimate of the charges before treatment has started.
Payment Policy
- Payment for services, including deductibles and co-payments, is due at the time of service unless prior arrangements are made.
- We accept the following payment methods:
- Cash
- Personal Check
- Credit cards (Visa, MasterCard, American Express, Discover). Please note a 3% surcharge applies to credit card transactions.
Patients with Insurance
Summit Oral Surgery is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. Final determination will be made by the insurance company once the claim has been processed. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. We will gladly discuss your treatment with you and answer any questions relating to your insurance.
You must realize, however, that:
- Your insurance is a contract between you, your employer, and the insurance company.
- Not every service is a covered benefit with all insurance contracts. Some insurance companies are selective in what services they cover.
- Services cannot be provided on the assumption that the charges will be paid by the insurance company; therefore, the patient is responsible for the bill, regardless of insurance coverage.
If a payment from your insurance company results in a credit balance, a refund will promptly be sent to you.
In order to maximize your benefits and because plans differ from carrier to carrier, and from policy to policy, our office may refer you to your carrier or employer’s benefits coordinator for assistance in understanding your plan. You are responsible for payment for all services regardless of the payment benefit.
If you have health insurance, you understand that the terms of your health insurance or health benefit plan(s) may reduce, limit or control what you are required to pay Summit Oral Surgery for the services you receive at Summit Oral Surgery. Whether or not you have health insurance, you agree to be financially responsible and pay for the services provided to you by Summit Oral Surgery if the services are not covered or fully paid for by Insurance and the law allows Summit Oral Surgery to collect from me the amount owing. You also agree to pay Summit Oral Surgery reasonable costs for collecting payments if you do not pay on time the amounts you are responsible for paying. These collection costs may include reasonable attorney fees whether or not legal action has been filed or appealed
Monthly Billing
- After finalizing claims with your insurance, we will send a monthly statement for any remaining balance.
- Payment plans may be arranged before treatment begins if necessary.
- Prompt payment is appreciated to avoid account delinquencies.
Outstanding Balances, Collections & Refunds
- Balances over 60 days may accrue a 1.5% monthly interest charge (18% annually).
- Accounts referred to a collection agency will incur an additional 35% fee of the overdue balance, plus legal and court costs if applicable.
- Any overpayment or insurance payments received after your balance is paid will be refunded promptly.
General Financial Policies
- We may refer you to your benefits coordinator for detailed information about your insurance plan.
- All charges must be settled regardless of insurance coverage.
